The toilet is usually the cleanest surface in the house. Flushing water followed by periods of dryness. Not a good surrounding for bacteria.
Same with the lid. You sit on it, but only a very short part of the day, the legs and buttucks are not that dirty of body parts and as soon as you stand up again the water quickly evaporates and the surface is again dry as the sahara.

So where is the problem with the ice machine then if we are exposed to it constantly anyway? By the relatively small number of people who actually get sick from the ice we can see it still actually is not that big of a problem, but the idea is that the bacteria in the machine can live and multiply and basically do a concentrated attack which then sometimes leads to problems compared to a constant low exposure.
